Health and social care services - Strategic Plan

Local NHS organisations and Somerset County Council have been working to develop a way forward for health and social care services in the County.
The strategic plans, known as Sustainability and Transformation Plans (STP) are being produced across the country and the Somerset plan has been put together by Somerset Clinical Commissioning Group (CCG), Primary Care and NHS providers and the County Council.LIt looks at the next five years, driven by the need for joined-up services that work better for our residents, are financially sustainable and have much greater emphasis on prevention.
